As our practice grows and patient needs evolve, this membership model allows us to provide ongoing care coordination, clinical oversight, and timely communication between visits—ensuring we can maintain the level of support that defines our care. The Kore Membership is now required to remain an active patient at Kore and provides essential support between visits.
The Kore Membership includes:
Personalized Medical Support
Prompt answers to questions related to current symptoms and active treatment plans (within 48 business hours)
Please note:
While timely care is a priority, excessive or repeated messaging may require a follow-up visit to address more complex needs, for Dr. Uppal's ability to fully help and make sense of the situation at hand.
New health concerns or review of new or external lab results require a follow-up appointment to ensure thorough and effective evaluation & treatment.
Prescription management between visits [including refills and modifications]
Adjustments to your supplement protocol between visits
Lab orders/requests between visits
Ongoing peptide care, including check-ins and dosage optimization
Administrative Support
Flex letters for insurance reimbursement
Superbills for insurance reimbursement
Written referrals and coordination of medical record requests

What the Kore Membership Does Not Include
To ensure clarity, the Kore Membership does not include the following:
Follow-up visits [these are billed separately at the current standard rate]
New patient consultations
Re-establishment visits, if membership is canceled or terminated due to payment status listed below.
Emergency or urgent medical care [we are not equipped to be either]
Acupuncture services; or any other services provided by any other medical professional
Discounts on any services other than what is listed above.
Pellets, nutrient/peptide injections, or treatments not listed under the membership
Peptides, in-office supplements, and medications (these are not covered under the membership fee)
Priority treatment or preferential scheduling — all active patients at Kore are required to be members, so no patient receives special access based solely on membership
Direct personal contact with Dr. Uppal or staff outside of clinic communication channels — while prompt responses are provided, membership does not include 24/7 access or personal messaging.
Payment Terms, Missed Payments & Cancellation
The Kore Membership fee of $125 is billed every 4 weeks to the card you provide.
Monthly membership payments are not eligible for superbills. However, you may use your HSA or FSA cards for payments.
It is your responsibility to ensure your payment method is current and valid by contacting Kore, via phone or text.
Kore does not accept insurance and is not contracted with, nor does it bill, any commercial insurance plans or federal healthcare programs, including Medicare/Medicaid.
Payment Failures
If your payment fails, we will attempt to charge your payment method again and reach out to you.
If we are unable to contact you or you have not updated your payment method by the second billing cycle, your membership will be paused until the outstanding payment is received and your billing information is updated.
If payment has not been resolved or we are still unable to reach you by the third billing cycle, your membership will be automatically canceled, and your status as an active patient at Kore will be terminated.
Cancellation & Hold Policy
If you cancel your membership, your patient status at Kore will end.
To resume care, you will need to re-establish as a new patient, complete the full intake process, and submit payment based on the pricing in effect at the time of re-establishment.
We understand that unforeseen circumstances can arise. You may place your membership on hold for up to one month per calendar year, with advance notice to Dr. Uppal or our front desk before the upcoming billing cycle.
Dr. Uppal reserves the right to terminate any Kore Member’s membership at any time.
If the patient is a minor (under the age of 18), the following terms apply:
Parental or Guardian Consent:
A parent or legal guardian must sign the Kore Membership Consent Form on behalf of the minor.
The parent or guardian will be responsible for ensuring that the membership fee is paid and for communicating any questions, concerns, or changes regarding the minor’s care.
Medical Support and Services:
The minor will receive all benefits included under the Kore Membership as applicable to their care. However, certain medical treatments or services may require parental consent or may be provided at the discretion of Dr. Uppal based on the minor's medical needs and current health status.
Account Management:
All communications, billing, and account management related to the minor’s membership will be handled through the parent or guardian’s contact information.
Membership Fee:
The Kore Membership fee is the same for minors as it is for adult members—$125 billed every 4 weeks
Cancellation Policy:
The minor's membership is subject to the same cancellation and hold policies as for adult members, as outlined in this agreement.
If the minor's membership is canceled, patient membership at Kore will end, and the minor will need to re-establish as a new patient if care is resumed, in accordance with the membership re-establishment terms.
Non-Refundable Payments and Service Usage
The Kore Membership is your responsibility to manage.
If you choose not to use the services provided, you are still required to pay the membership fee.
You are not entitled to request a refund for any unused services, regardless of the length of time the services were not used.It is your responsibility to cancel your membership and prevent future billing cycles if you no longer wish to continue.
To stop future charges, you must contact Dr. Uppal or our patient services team before the next billing cycle.
No refunds will be issued for any past payments, regardless of whether you used the services during that time.
Note:
If your Kore Membership is no longer active, you may still book shockwave treatments separately and continue accessing your online supplements through Fullscript.
However, you will no longer receive the 15% Fullscript Member discount, guidance on supplements or protocol changes.
You acknowledge that you are taking all supplements at your own discretion and assume full responsibility for your health and safety.
